Medical Binder Printables
Time is valuable to both you and your doctors. Make that time as efficient as possible by creating a Medical Binder with Medical Binder Printables containing all of your important medical information.
After being diagnosed with Hashimoto’s a few years ago, my journey to health included visits to new specialists. Of course, each new specialist wanted to hear all about my medical history. They expected me to remember every prescription I had an allergic reaction to, surgery dates, and every supplement and dose I was taking.
After digging through loads of medical files for the umpteenth time, I decided there had to be a better way.
Why, yes, I did try Selenium. I started taking it on January 17, 2015, and stopped on April 23, 2016, because it was doing nothing for me. I have the labs to prove it, right here! That’s empowerment!
Carry your Medical Binder to each appointment and you can be empowered, too!

Purchase The Medical Binder Here!
The Medical Binder comes in two versions, one regular and one fillable.
What is a fillable PDF?
If you don’t want to fill your information in by hand or if your writing is ineligible, a fillable document is perfect for you! When you open a fillable PDF in Adobe, the fields can be filled out, right on your computer. The editable PDF uses fillable form fields for your convenience. The fillable fields are laid over a pre-made template which can’t be altered. I recommend opening the file in Adobe Acrobat PDF Viewer for the most reliable results. If using a Mac, please open in Adobe PDF Reader as the Mac Preview software does not work well with fillable fields.

Thanks for this!
I have 5 dogs and 7 cats. As the dogs get older, more and more little medical issues seem to pop up, so this will be fantastic for my own records and for the doctors if we should ever have to see a new vet/specialist.
Hey! What a great idea! It’s sounds like you have a super fun home full of lots of love and what an awesome way to keep all of your medical records straight!
I have found that getting copies of the actual dr notes and including them in the respective section is a big help. I also get a copy of any test results, such as bloodwork, CT scans, MRI’s. This way doctors can make copies of what they need and get it quicker.
Excellent advice, Lynn!
I have a disabled daughter. we are always at a doctor visit or hospital visit or testing. I would love to see a medical binder with blood pressure tracker, O2 levels tracker, Doctor visit, hospital visit. Med List, Medical bill tracker, scheduled doctor appointments, insurance records, medical address list and calendar. Being able to carry a binder with all necessary items needed would be such a blessing. Also adding the cover and spine.
Awesome! This is a gold mine of ideas that I LOVE! Thank you so much for your input, Denise!
This is phenomenal! My husband suffered through a “mysterious illness” a year ago and ended up creating something similar, but this would have been waaaaaay handier. Awesome! P.S. – We’ll have to chat Hashi’s someday. Crazy how common it’s becoming.
Janeen, do you have Hashimoto’s too? It is crazy how common it is.
I do. Thankfully it’s never been a huge deal for me (I started taking meds when I was a teen, didn’t realize that non-meds were even a possibility). My twin brother was only diagnosed this last year and it’s been tough going for him trying to get it under control. It sounds like you’ve had a similar experience. I think he’s created something similar to your binder too, and it’s helping him figure things out. I hope you get to feeling awesome soon!
Thank you, Janeen! We’re getting there, one step at a time!
Thanks so much. I have needed something like this for awhile . Cann’t wait to get started.
Awesome! I know you’re gonna LOVE it!
Thank you! My husband and I have been in need of this. Please keep them coming.
That makes my heart HaPpY! Thank you, Patti!
WOAH! That was amazing Bridget ~ very comprehensive! ๐
But wait, so a dentist DOESN’T need to know about a wart on your toe? Now you tell me! ๐ Man, you’re a hoot Bridget! ๐
TeeHee! Kay, I’m thinking YOUR dentist would love to hear about a wart on your toe:) After all, you are an excellent storyteller!
